Specially trained in end-of-life care, our experienced team of hospice professionals help patients live out their remaining days in comfort and dignity. Our nurses and physicians are well versed in symptom management and pain relief, and our compassionate aides provide companionship and personal care. We also offer emotional support for patients, their families, and the entire community through our Bereavement Services.

Private Duty

When everyday activities like bathing, dressing, laundry and shopping are a challenge, a specially trained Pathways Private Duty aide is a welcome solution. Private Duty Care is non-medical, and customized to each individual’s needs, and is available up to 24 hours a day, on a temporary or long-term basis, at a home or in assisted living.

Geriatric Care Management

The responsibility of overseeing a senior’s care is a serious challenge when you live at a distance or are juggling work, kids, and other responsibilities. Our Care Management professionals can help you evaluate a senior’s needs, access local services and support systems, and make a plan that’s right for your family. Many families use this service in conjunction with Pathways Private Duty services.

Join Pathways for a unique opportunity to play the challenging Los Altos Golf and Country Club course with friends, colleagues, or clients while supporting Pathways Home Health & Hospice.

Originally designed in 1923 by Tom Nicoll, the 18-hole, par 71 course underwent a complete renovation in 2004. Meticulously maintained, it is one of the most beautiful courses in the Bay Area.

Space on the course is limited, so if you would like to play, reserve sponsorships that include foursomes early!
