We work with clients in Alameda, western Contra Costa, San Francisco, San Mateo, and Santa Clara Counties. Our commitment to helping the elderly, healing the sick, and comforting the dying has earned us the trust of physicians and families throughout the Bay Area. Home is the most comfortable and comforting place to heal when you are dealing with a health challenge. Pathways Home Health makes that possible with comprehensive services for people who are recovering from an illness or surgery, managing a chronic disease, or coping with a life-threatening condition. Our skilled team helps patients improve their health so they can get back to enjoying the best quality of life possible.
Specially trained in end-of-life care, our experienced team of hospice professionals help patients live out their remaining days in comfort and dignity. Our nurses and physicians are well versed in symptom management and pain relief, and our compassionate aides provide companionship and personal care. We also offer emotional support for patients, their families, and the entire community through our Bereavement Services.
When everyday activities like bathing, dressing, laundry and shopping are a challenge, a specially trained Pathways Private Duty aide is a welcome solution. Private Duty Care is non-medical, and customized to each individual’s needs, and is available up to 24 hours a day, on a temporary or long-term basis, at a home or in assisted living.
The responsibility of overseeing a senior’s care is a serious challenge when you live at a distance or are juggling work, kids, and other responsibilities. Our Care Management professionals can help you evaluate a senior’s needs, access local services and support systems, and make a plan that’s right for your family. Many families use this service in conjunction with Pathways Private Duty services.
