Pathways Fall Prevention - Safety Checklists

As a community-based, not-for-profit organization, Pathways is dedicated to providing education and support to patients and families in the communities we serve.

Pathways saw a need to produce easy to read flyers to increase awareness about fall prevention hazards in and around the home. The Fall Improvement Taskforce (FIT) at Pathways created these colorful flyers that can be printed and used as check-off lists to ensure your environment remains safe and to minimize the risk of falls.
