Let us introduce you to the Pathways program that can help patients with Heart Failure be Heart Sucesses!

Some homebound patients need the support of Home Health to be successful at heart failure management. Our nurses have specialty training in heart failure to support patients during that vulnerable period between an acute episode and independence. With our coaching, patients transition from being cared for to caring for themselves.

Our program targets:Heart Success

  • Medication reconcilation and management
  • Self care, such as a low sodium diet, daily weights and increase exercise
  • Teaching "red flags" to watch for and report
  • The importance of follow-up visits to health care providers

We use plain language, a simple personal health record, careful monitoring, and lots of teaching to help patients achieve their healthcare goals. Our program is patient-centered, with patients and their families actively participating in their care. Our care can prevent re-hospitalization by teaching patients how to avoid heart failure hazards.

We inspire in our patients the confidence to manage their heart health independently!

Arranging Home Health Care

If you are hospitalized, talk with your doctor, nurse or hospital social worker about home health care and ask for Pathways as your home health provider. We will work closely with them to provide a smooth transition from hospital to your home.

If you are not in the hospital you can talk with your doctor or with our admissions staff. We will work with your doctor to develop a plan for your home health care. Our insurance coordinator will verify benefits for you.
