House Call re-Admission Reduction Program (HARP)

What is HARP?

The HARP program will help you with your discharge from hospital to home. You will receive care from Pathways Home Health, affiliated with El Camino Hospital since 1986, and Dr. David Jones, a physician who specialized in making house calls to seniors. Dr. Jones and the Pathways care team will visit you at home and coordinatre your care. We will work with you to keep you health at home and help you get back to your regular doctor.

Who do I call with my medical questions and concerns?

• You will call Pathways Home Health at 1.888.755.7855, 24 hours a day, seven days a week.

What about my regular doctor?

• Our goal is to get you back to seeing your regular doctor over the nest few weeks. You can decide at any time that you are ready to return to your regular doctor—just let us know.

What is paying for this service?

• You regular Medicare benefit and an El Camino Hospital community grant.
