The Pathways KIDS Team

The Pathways KIDS team consists of several healthcare professionals that will teach your family and help you find the support you need to give your child the best care.

We give parents the confidence to care for seriously ill children at home. We become part of the medical team you already have—the part that sees you at home. We are always just a phone call away, available 24 hours a day.

You are in charge of the care we give, and can choose as many of our professionals as you need or want. In addition to your nurse case manager, your Pathways KIDS team may include:

• Palliative care doctor
• Pediatric nurse
• Medical social worker
• Home health aide
• Pharmacist
• Spiritual care counselor
• Volunteers
• Bereavement counselors

A team member is available to visit you at home or in the hospital to give you information. We may also participate in hospital care conferences.
