Pathways Programs

As a community-based, not-for-profit organization, Pathways believes in developing innovative programs to serve the needs of the communities we serve. With the help of generous community support to Pathways Hospice Foundation, we are proud to offer the following programs to our patients and their families.

Palliative Care

At Pathways we respect the individuality of our patients and families, and we know that their approaches to serious illness may vary. The Palliative Care Team recognizes the difficult transition patients and families must make as cure becomes less likely, and the focus of care begins shifting to comfort measures.

Transition Care & Chronic Disease Support

Homebound patients that are learning to manage their care at home can benefit from the support and education of Home Health care services from Pathways. Our care teams are specially trained to support patients during that vulnerable period between an acute episode and independence. With our coaching and medication management, patients transition from being cared for to caring for themselves.

Pathways KIDS - Palliative Care for Children

Pathways KIDS is a family-centered program specifically designed to provide home visits for children with life-threatening illnesses. The focus is on enhancing quality of life for the child and family through the medical expertise of the Pathways KIDS Team and its supportive care. The team is on call 24 hours a day for consultation or home visits when needed.

Integrative Therapies

Integrative Therapies include a variety of different physical, emotional, and spiritual modalities, including complementary and alternative therapies such as massage, energy work, and guided imagery in addition to more conventional Western medicine.
