Sequoia Healthcare District Awards $60,000 to Pathways Home Health & Hospice

Funds to support Chronic Care Programs for At-Risk Elders

SUNNYVALE, CA – December 7, 2011 – Pathways Home Health & Hospice received a grant for $60,000 from the Sequoia Healthcare District. The grant will support the creation and management of Chronic Care Management Programs for at-risk seniors and patients residing within the Sequoia Healthcare District. These communities include Atherton, Belmont, Menlo Park, Portola Valley, Redwood City, San Carlos, Woodside and parts of Foster City and San Mateo.

Silicon Valley Business Journal Honoree

SUNNYVALE, CA. – December 2, 2011 – Satu Johal, CFO at Pathways Home Health & Hospice, received the Silicon Valley Business Journal 2011 CFO of the Year Award in the Nonprofit Company Category. The CFO of the Year Awards were established by the Silicon Valley Business Journal to recognize CFOs for their part in the success of their enterprises. Johal was honored for her ability to navigate current economic challenges and for her skill in dealing with a myriad of financial regulations.

Pathways Home Health & Hospice Hires Carolyn Bonner as Vice President of Hospice

SUNNYVALE, CA, September 6, 2011 – Pathways Home Health & Hospice welcomes Carolyn Bonner to the position of Vice President of Hospice. Bonner, a registered nurse, brings over 35 years of managerial experience in the hospice and home health industry to her new position. She recently held the position of Area Administrator for Kaiser Permanente in San Diego for 11 years. Prior to that, Bonner was the Director of Home Health and Hospice at Providence Home Health & Hospice in Seattle.

Pathways Hospice Honors Bill Jennings with Kennedy Award in Recognition of Volunteer Service

Funds to Support Pathways KIDS Pediatric Program

SUNNYVALE, CA – March 23, 2011 – Pathways Hospice Foundation recently received a grant for $25,000 from the Sharks Foundation. The grant was one of 11 awarded by the Sharks Foundation to community-based, not-for-profit organizations that serve youth and children in the Santa Clara County. The grant received by Pathways Hospice provides for health care and home visits for children with life threatening illnesses. The Sharks Foundation grant will enhance the quality of life for children and their families through the medical expertise of the Pathways KIDS team.

Pathways Hospice Continues to Make History with University Mound Ladies Home

Sunnyvale, CA - March 16, 2011 - Pathways Hospice and University Mound Ladies Home (UMLH) recently partnered to reunite Lloyd Paff with his wife LaVerne, to whom he was married for 68 years. Lloyd, a San Francisco native, was the first man to be admitted to UMLH in its 127 years of serving San Francisco’s elderly. LaVerne and Lloyd lived in separate assisted living facilities until January 23rd, when they were reunited after being apart for more than three months.

SUNNYVALE, CA – February 24, 2011 – Pathways Hospice Foundation recently received a grant for $85,000 from the Washington Hospital Healthcare Foundation. The grant will support hospice care and services to patients residing within the Washington Township Health Care District, which includes Fremont, Newark, Union City, part of South Hayward, and unincorporated Sunol.
