Crisis Comfort Care Program

Crisis Comfort Care nurseCrisis Comfort Care (Continuous Care)

Pathways has expanded our ability to provide the hospice “continuous level of care.” This is for people who need extra nursing hours to control their symptoms. We have named our newly expanded program Crisis Comfort Care.

To meet the needs of even more patients we have hired a team of nurses covering all shifts and all geographic areas that are dedicated to providing Crisis Comfort Care. All visiting staff have been trained to identify early in the process those patients whose increasing needs require a period of extra hours of bedside care.

Medicare regulations state that Crisis Comfort Care can be provided by RNs, LVNs or in part by hospice aides. The following is a description in the regulations about when this level of care can be started.

Sec. 418.204 Special coverage requirements: Periods of crisis. Nursing care may be covered on a continuous basis for as much as 24 hours a day during periods of crisis as necessary to maintain an individual at home.

A period of crisis is a period in which the individual requires continuous care to achieve palliation or management of acute medical symptoms.

We are excited to be able to provide this exceptional level of care to more patients than we ever have before.

Arranging Hospice Care

Talk with your doctor or health care provider about hospice care and ask for Pathways as your hospice provider. We will work closely with them to provide a smooth transition to hospice. When it's time for hospice care, remember you're not alone...we're here to help.
