Home Health Patient and WifeRehabilitation at Home

Pathways Home Health Care provides skilled physical and occupational and speech pathologists for patients at home, allowing shorter hospital stays and safer transitions to home.

Nursing, social work, and home health aides can round out the patient’s care when these services are also needed. During their visits our professionals provide hands-on care, and teach patients and caregivers what they need to know for optimal health and managing care at home. One of our goals is to prevent re-hospitalization.

Program Features

Pathways provides admissions, treatments, and consults Monday through Saturday. Our hospital liaisons provide a smooth transition home. Patients have one therapist case manager to coordinate all the patient’s needs. Our therapists have specialties in a wide range of fields. We have effective working relationships with many equipment providers to ensure good customer service.

Therapy Options

Pathways’ rehabilitation specialists provide an extensive array of services for:

  • Stroke and head trauma
  • Fractures and joint replacement.
  • Acute and chronic back care
  • Gait, transfer, and mobility problems
  • Degenerative neurological diseases
  • Musculoskeletal diseases
  • Spinal cord injuries
  • Sports injuries
  • Swallowing difficulties and dysphagia
  • Speech and language problems
  • Laryngectomy or glossectomy
  • Cardiac and pulmonary disease

Our therapists can also evaluate a patient’s risk for falls, home safety, and need for adaptive aids. They may instruct about topics such as independent living skills, body mechanics, energy conservation, and exercise programs.

Arranging Rehab at Home

If you are hospitalized, talk with your doctor or hospital case manager about home care and ask for Pathways as your home health provider. We will work closely with them to provide a smooth transition from hospital to your home. If you are not in the hospital you can talk with your doctor or with our admissions staff. We will work with your doctor to develop a plan for your home health care. Our insurance coordinator will verify benefits for you.
