Pathways Memorial Service
Evening of Remembrance 2013

Pathways Home Health and Hospice invites you to remember a loved one, in the company of others, as we honor those who have died this past year.

The evening will include a short program with music, readings, and support. It will also feature the creation of a magnificent collective bouquet and a video montage.

To include a photo of your loved one in our video slide-show, please email us a photo (in jpeg, tif, or gif format) to: bereavement@pathwayshealth.org. Please submit a digital photo by April 23rd.

If you would like to bring a picture or memory object of the person you are remembering, please do so.

We will have flowers available, but please feel free to bring a special or significant flower in honor of your loved one to place in the bouquet.

Light refreshments will be served following the programs.

Please RSVP to the Bereavement Program to help us plan for all who wish to attend. Call 1.888.905.2800 x 4241 or email: bereavement@pathwayshealth.org. Indicate which date and location you plan to attend and how many will be attending. However, please attend even if you have not formally notified us.
